In order to participate, stores must sign the Book Sense Gift Card Participation Agreement, the Gift Card ACH Agreement, and submit the Gift Card Program Order Form. Please use the link provided below to download a PDF document. All pages must be completed and returned as instructed.

Use the above link to download and print a copy of the PDF. The file contains three documents (1) Book Sense Gift Card Program Participation Agreement, (2) Gift Card ACH Agreement, and (3) Gift Card Program Order Form.

The Participation and the ACH Agreement contain important information for your review. Both documents require an authorized signature that is kept in a secure file.
